Overview:
This affidavit is required for parents in Lithuania applying for their minor child's passport in India when the other parent(s) is/are in Lithuania. It certifies that the parent(s) in Lithuania have no objection to the issuance of the passport to the minor child.
Requirements:
a. Miscellaneous Service form duly signed with one passport-sized photograph on top and the right-hand side of the Form.
b. For Indian Passport Holders: Valid passport in original and one set of self–attested copies of the first two and last two pages of the passport and the page(s) containing passport officer’s observations (if any).
c. For Other Passport Holders: Valid passport in original and one set of self-attested copy of the passport page(s) containing personal particulars of the passport holder.
d. Proof of residence: A self-attested copy of a utility bill (telephone/electricity/gas/water or council tax bill), or Driving license, or lease deed of residence, etc., where the applicant's Lithuanian address is clearly mentioned.
e. Visa Status: Self-attested copy of the passport page with a valid visa/Valid Lithuanian Residence Permit showing the visa status.
f. Sworn Affidavit in the prescribed format, filled in by the parent(s) living in Lithuania. The Affidavit needs to be signed before the Consular officer. A separate form is required for each child.
g. Fee: Euro 19 + Euro 2 (ICWF)

Please Note:
The applicant may be advised to submit additional documents after scrutiny by the Consular Section on a case-to-case basis. The applicant needs to appear in person with a valid passport and sign before the Consular officer.
If there is more than one document, the fee should be paid accordingly. Fees once tendered are non–refundable.
