Registration of Birth of a Minor Child and List of Documents to be submitted along with the application:  

1. Fill the online application form on Govt of India's web portal.
2. Upload / attach the requisite documents with the web application filled online on the above portal.
3. After filling up the form and uploading the above-mentioned documents, print out the form and have it duly signed by both parents.

4. Please visit the website: (https://indianconsularservices.mea.gov.in/consularServices/) for availing various Miscellaneous Consular Services through Online Consular Services System (SEWA Portal).
5. Please Sign up and create your account using the above weblink. Activation of your account will be done by using OTP received on your registered email ID. Once your account is activated, please login to your account and apply for the required miscellaneous consular service. Users can avail multiple services with the same account. Users may follow each step and fulfill the requirements of online application process including uploading the documents and submitting the same on the portal.
6. On successful submission of the application, a provisional acknowledgment receipt will be generated which the applicant can print after login to his/her account.
7. After scrutiny of the documents by the Mission/Consulate, the applicant will receive a confirmation message regarding submission of physical application. It can be seen in the dashboard of his/her account.

Noteworthy points

8. In addition to uploading documents at SEWA portal, following documents are required in original plus one copy each during submission at the Embassy:
9. Three recent colour photographs of the applicant/child, measuring 35 mm × 45 mm, with a plain white background (Photo specification details  https://www.eoivilnius.gov.in/wn-28-11-2025.php).
10. Copy of  copy of the Birth Certificate issued to the child by the Lithuanian hospital, Gimimo įrašą liudijantis išrašas / Extract from the local government office, Child's personal identification number (Asmens kodas). Original document must be presented for verification.
11. Copy of Proof of Local Residence of parents, original document must be presented for verification.
12. Copy of Passports of Parents. Original passport of parent(s) must be presented for verification, self attested copy of Voter Id card/Aadhaar card/Driving license etc. may be submitted.
13. Copy of Valid Residence Permit of both parents, original documents must be presented for verification.
14. Copy of Indian Marriage Certificate duly attested/apostilled by Ministry of External Affairs, New Delhi and copy of Marriage Certificate should be signed by both parents Note: Marriage certificates must be apostilled or otherwise authenticated/certified by the competent authority of the issuing country, or certified by the relevant Embassy/Consulate. Marriage certificates in languages other than English must be translated into English by an authorized translator. The original certificate must be presented for verification. Indian marriage certificates issued by a Muslim Jammath or other non-government department must be notarized by the MEA in India.

• The presence of both parents is mandatory at the time of submission of the application.

• Please bring all the suggested original documents for verification.

SECOND STEP 
(APPLICATION FOR PASSPORT): Following documents are needed to apply for fresh passport to a new born child:

• Procedure as mentioned above in First Step for registration of child as an Indian citizen be followed and separate set of documents may be attached as mentioned above.
• Printed copy of the duly filled passport application form at Global Passport Seva Online 2.0 Portal
• Instead of signature, thumb impression of the child is required in signature box on the first page. Please note that the thumb impression must be clearly visible and within the designated box. The last page of the application form should be signed by the parent(s).
• Copy of  copy of the Birth Certificate issued to the child by the Lithuanian hospital, Gimimo įrašą liudijantis išrašas / Extract from the local government office, Child's personal identification number (Asmens kodas) including the copy of Birth registration issued by the Embassy.  Original document must be presented for verification.
• Three recent colour photographs of the applicant/child, measuring 35 mm × 45 mm, with a plain white background. The face should cover 85% of the photograph. 
• Self-attested copies of parents' passports and residence permits / visas, Lithuanian address proof, and self attested copy of Voter Id card/Aadhaar card/Driving license etc. Original document must be presented for verification.
• Consent Letter / Affidavit from parents / guardian. Click here to download Annexure-D. Presence of the child and both the parents is mandatory Embassy of India, Vilnius at the time of submission of the application.
• All documents related to minors or newborns must be self-attested by both parents.
• If the applicant is a newborn or a minor, the parents need to bring a copy of Indian Marriage Certificate duly attested/apostilled by Ministry of External Affairs, New Delhi and copy of Marriage Certificate should be signed by both parents Note: Marriage certificates must be apostilled or otherwise authenticated/certified by the competent authority of the issuing country, or certified by the relevant Embassy/Consulate. Marriage certificates in languages other than English must be translated into English by an authorized translator. The original certificate must be presented for verification. Indian marriage certificates issued by a Muslim Jammath or other non-government department must be notarized by the MEA in India.
• If one of the child’s parents is a foreign national, the applicant may be required to submit a notarised (Lithuanian) declaration confirming that the minor child has not acquired any foreign citizenship or been issued a foreign passport.
