Procedure of change of Name/Address/Surname for Adult

If any of the applicant’s personal details have changed since the issuing of the current passport (i.e. change of name and change of printable address in the passport), you will need to submit additional documents. When a new/re-issue of the passport is being issued, no additional fee is charged for a change of address and for a change of name.

Documents required :

  • Duly filled and printed online application form (https://embassy.passportindia.gov.in/) with two ink signatures (first page below the photo and last page in the signature box). Kindly mention the proper Indian address in the application form for the purpose of Police verification (two addresses are required in the application form, one address should be from India and the other/current address in the foreign country).
  • Three square-shaped colour photos, size 2 inches x 2 inches (51 mm X 51 mm). For more information, please see: [photo spec passport pdf]
  • Self-attested copies of the first and last page of your current passport {Current passport (Mandatory)}
  • Self-attested copies of your Visa/ residence permit/TRP (both sides)
  • Additional documents required for change of printable INDIAN address in the passport:

Proof of the new address such as a copy of your voting card/Aadhaar card/driving license/electricity bill, bank statement, rent agreement, etc.

  • Additional documents required for change of printable FOREIGN address in the passport:

 Proof of the new address such as a copy of electricity bill, telephone bill, bank statement, rent agreement, etc.

  • Additional documents required in case of a change in Name:
  • Documents required from India (Thi would not be applicable for temporary residents and visitors from India or third countries):
  • Clippings of two local newspapers or the Gazette notification of the concerned State Government, as the case may be; and
  • At least two public/school documents issued in the desired name to ascertain that the applicant has actually changed his name.
  • OR/Alternatively Documents required from Lithuania:
  • For those who have been staying abroad on long term permanent or work, student visa/permit, two public documents issued by the local (Lithuania) government/local school/educational institution where the student is enrolled may be accepted.
  • Regarding SURNAME – If any applicant wants to add/shift surname from the given name, may be allowed subject to the production of at least two public documents in the said name. For example, if an applicant held a passport in the name of SANJU MOHAN PRASAD with the SURNAME blank, he may be allowed to enter SANJU MOHAN or SANJU as the FIRST NAME and PRASAD or MOHAN PRASAD as SURNAME.
  • Change/add in Surname subsequent to marriage

If your surname has changed subsequent to marriage and you want to apply for a new passport in your married name, you will need to submit an affidavit for change/add in surname ([Affidavit-for-change-of-name-after-marriage.doc]  together with your passport application. You are also required to furnish the following additional documents:

  • Copies of your spouse’s passport/Aadhar Card/Voting Card.
  • Proof of payment (Fee-receipt)
  • Payment by bank transfer only.  All fees paid for this service are non-refundable
  • Please make separate payments for Consular Fees and Consular Surcharge/ICWF using the following bank details and provide two separate receipts; one for Consular Fees and another for Consular Surcharge/ICWF.

b.  MINOR CORRECTION IN NAME AND SPLITTING OF NAME INTO GIVEN NAME AND SURNAME

The following cases of minor correction in name and splitting of the name will not be treated as 'name change cases' and will be considered as under:-

      • Spelling correction/change of spelling which phonetically does not result in change in name (e.g., D’souza to De Souza);
      • Variation of a name as mentioned in Birth Certificate (e.g., Sanjiv to Sanjeev)
      • Shifting of words/changing arrangements of various parts of the existing name without inserting/deleting any words /letters in the existing name (e.g., Parmeshwaran Shiva Kumaran to Shiva Parmeshwaran Kumaran). The most recent name as shown in at least a couple of personal documents, educational certificates, etc. issued by Government authorities are to be submitted in such cases.
      • Splitting of the given name into the first name and surname may be allowed subject to the production of at least two public documents in the said name

Applicants are advised to follow the above-mentioned steps and submit the application along with all the relevant documents. Incomplete applications due to any missing relevant documents cannot be processed, and it can take more than the usual processing time..
