Life Certificate for Pension Purposes or for Any Other Purpose

Overview:
An individual requiring a Life Certificate for drawing a pension in India or any other purpose can apply for this service. The application must be made in person.

Requirements:
a. Miscellaneous Service form duly signed with one passport-sized photograph on top and the right-hand side of the Form. [Link]
b. For Indian Passport Holders: Valid passport in original and one set of self–attested copies of the first two and last two pages of the passport and the page(s) containing passport officer’s observations (if any).
c. For Other Passport Holders: Valid passport in original and one set of self-attested copies of the passport page(s) containing personal particulars of the passport holder.
d. Proof of residence: A self-attested copy of a utility bill (telephone/electricity/gas/water or council tax bill), or Driving license, or lease deed of residence, etc., where the applicant's Lithuanian address is clearly mentioned.
e. Visa Status: Self-attested copy of the passport page with a valid visa/Lithuanian Residence Permit showing the visa status.
f. Original Pension Book plus a photocopy if the life certificate is required for pension purposes.
g. Fee:
- For Pension Purpose – Gratis (Only 2 Euros will be charged on account of Consular Surcharge).
   - Other than pension purpose: Fees - Euro 19 + Euro 2 (ICWF)

Normally, the same day.

Please Note:
The applicant may be advised to submit additional documents after scrutiny by the Consular Section on a case-to-case basis. The applicant needs to appear in person with a valid passport and sign before the Consular officer.

 If there is more than one document, the fee should be paid accordingly. Fees once tendered are non–refundable.
