Certificate for Transporting Human Ashes to India

Documents Required:

a. Miscellaneous Application Form and Death Registration Form (for Indian passport holders), filled and signed by the applicant.
b. Two Photographs (2”x2”).
c. For Indian Passport Holder:
- Valid passport (original) and self-attested copies of the first two and last two pages, and pages containing officers' observations (if any).
d. For Foreign Passport Holder:
- Valid passport (original) and self-attested copies of the passport pages containing personal particulars.
e. Proof of Residence of the applicant: Self-attested documents like a copy of a utility bill, Driving License, lease deed, etc., with the Lithuania address.
f. Death Certificate (original) of the deceased issued by the relevant local authority and its photocopy.
g. Passport copy of the deceased person.
h. Original passport of the deceased (for cancellation if an Indian passport holder), Original OCI/PIO card (for cancellation if the deceased had obtained these cards).
i. Cremation Certificate (original) of the deceased and its photocopy.
j. Certificate from the Funeral Director/Undertaker confirming the urn contains ashes/mortal remains, indicating the name, age, date, time, and cause of death of the deceased.
k. Fees: Euro 19 + Euro 2 (ICWF)

l Processing Time:
- Same day if applied in person.
- If applied by post, Minimum 3 to 5 working days (excluding the submission and dispatch dates) subject to the application being complete.

Note:
- Original documents provided will be submitted along with photocopies and returned once the process is over.
- Additional documents may be requested on a case-by-case basis after scrutiny by the Embassy.
