Checklist/Order of Documents for Power of Attorney/Affidavit/Will/Gift Deed:
a. Miscellaneous Service form duly signed with one passport-sized photograph on top and the right-hand side of the Form. [Link]
b. Original and one copy of Executant(s) applicants' Valid Passport(s) (Copy must be self-attested). If an Indian Passport holder, copies of the first two pages and last two pages duly self-attested.
c. Self-attested copy of passport page with a valid visa/valid Residence permit card showing the visa status should be enclosed.
d. Original and one copy Executant’s/applicant’s proof of address.
e. Original and one copy of two witnesses’ Valid passports with valid visa copies (Residence permit/TRP) and copies of their proof of address.
f. Original and one copy of Power of Attorney with one passport-size photograph pasted on the last page of PoA near the place of Executant's signature.
For attestation related to Power of Attorney/Affidavit/ Will/Gift Deed, the applicant shall appear in person to submit their application. In the case of Joint Power of Attorney, all the deponents shall present themselves before the Consular officer of this Mission.
Details of Documents required:
a. Completed Miscellaneous Services Form
b. In case of Indian Passport Holders: Valid passport in original and one set of self–attested copy of the first two and last two pages of the passport and the page(s) containing passport officer’s observation(s) (if any).
ii. In case of Other Passport Holders: Valid passport in original and one set of self-attested copy of the passport page(s) containing personal particulars of the passport holder.
c. Proof of residence: Any of the following self-attested documents, viz., a copy of utility bill (telephone/electricity/gas/water or council tax bill), or Driving license, or lease deed of residence, etc., where the applicant's Lithuanian address is clearly mentioned, should be provided.
d. Visa Status: Self-attested copy of the passport page with a valid visa/Valid Lithuanian Residence Permit showing the visa status should be enclosed.
e. Witnesses: Power of Attorney (PoA) needs to be witnessed and signed in person before the Consular officer by two persons known to executants. Names, Lithuanian address, and valid passport particulars such as passport number, date of issue, and place of issue need to be clearly mentioned in the PoA. Witnesses need to produce their original passport at the time of signing of the PoA, and one set of self-attested copy of the passport pages containing personal particulars of the witnesses need to be submitted along with the PoA. The witnesses to the PoA should not be amongst the beneficiaries of the PoA.
f. i. For attestation of Power of Attorney Document: Recently drafted Power of Attorney (PoA) can either be on plain paper or stamp paper as per your requirement or as advised by your lawyer, and one set of the copy needs to be signed by the applicant before the Consular officer. Executant(s) of PoA are also required to affix one passport-size photograph on the last page/at the end of PoA near the place for his/her signature as an executant(s) and then make a photocopy of the PoA for retention by the Embassy.
ii. For attestation of Affidavit: Recently drafted Affidavit can either be on plain paper or stamp paper as per your requirement or as advised by your lawyer, and one set of the copy needs to be signed by the applicant before the Consular officer. Executant(s) of Affidavit are also required to affix one passport-size photograph on the last page/at the end of Affidavit near the place for his/her signature as an executant(s) and then make a photocopy of the Affidavit for retention by the Embassy.
iii. For attestation of Will/Deed Gift: Recently drafted Will/Deed Gift can either be on plain paper or stamp paper as per your requirement or as advised by your lawyer, and one set of the copy needs to be signed by the applicant before the Consular officer. Executant(s) of Will/Deed Gift are also required to affix one passport-size photograph on the last page/at the end of Will/Deed Gift near the place for his/her signature as an executant(s) and then make a photocopy of the Will/Deed Gift for retention by the Embassy.
g. Affidavit: In the case of Affidavit(s), No witnesses are required. However, other procedural and documentation requirements as prescribed above would be required to be complied with.
h. For Court cases pending in India – Applicants have to provide a copy of the Court case to be lodged or pending against the executants.
i. Fees: Euro 19 + Euro 2 (ICWF)
Please make separate payments for Consular Fees and Consular Surcharge/ICWF using the following bank details and provide two separate receipts; one for Consular Fees and another for Consular Surcharge/ICWF.

Note: The full name of the applicant and the service applied for must be displayed on the printed receipt and cannot be added by hand.
Please note that the fee is for each signature and stamp of the consular officer.

Please Note: The above are the general mandatory requirements. The applicant may be advised to submit some more documents after scrutiny of the application and documents at the Consular Section, on a case-to-case basis.
If there is more than one document that needs to be attested, the fee should be paid accordingly. Fees once tendered are non–refundable.
